Declaration of Acknowledgement of Risk

I understand that it is a condition of my entry into the Hemel Hempstead Leisure Centre Outdoor Swimming Pool I declare the following for the benefit of A1 SUP Hire and the site managers:

1. I am:

(a) At least 16 years of age.

(b) In good health.

(c) A regular swimmer at the Hemel Hempstead Leisure Centre Outdoor Swimming Pool or other unheated open water swimming venues.

(d) Habituated to cold water swimming.

2. I am aware that swimming in open water involves a risk of hypothermia and/or sudden

immersion syndrome (SIS) and/or cold-water shock.

3. I acknowledge that I have been warned by A1 SUP Hire of the risks associated with swimming in cold, untreated and opaque water.
